Investigation of Women's Lived Experiences of Cigarette Smoking: A Qualitative Study

Introduction: Society has traditionally perceived tobacco use as predominantly a male issue. However, the lack of awareness among women about the effects of smoking has led to an increase in cigarette smoking prevalence among women in recent years, with detrimental impacts on both physical and mental health, affecting those around them as well.
Objective: This study aimed to identify the reasons for initiating and continuing cigarette smoking among women, as well as to evaluate associated challenges.
Methods: This qualitative research was conducted using a phenomenological strategy in 2022. The research population consisted of women cigarette smokers in Tehran. A purposeful and convenience random sampling method was used among women who frequented cafes and traditional tea houses in Tehran. Data were collected from 10 participants until theoretical saturation was reached and analyzed using Colaizzi's method.
Findings: The results indicated that internal factors, including individual needs, feelings, and psychological emotions, were primary reasons for cigarette smoking. Major interpersonal issues originated from family and friends. Societal issues included potential challenges in academic environments, relationships with friends, and social patterns. Cultural issues referred to literacy levels and media influence, while economic issues were categorized without further classification, all of which were significant reasons for cigarette smoking. Reasons for the continuation of cigarette smoking among women smokers included a lack of sufficient reasons to quit fear of quitting, and lack of motivation to stop smoking. Additionally, the problems related to cigarette smoking among women were dentified as emotional indifference and numbness towards surrounding events, and the problems resulting from continued cigarette smoking included fear of disapproval from supporters, especially fathers and family, fear of causing harm to supporters, and concerns about losing health and beauty.
Conclusion: The role of family and friends is crucial in the initiation and continuation of cigarette smoking among women. Therefore, it is recommended that mental health policymakers, including educational institutions, organize workshops aimed at educating parents and familiarizing them with adolescence. Social institutions should address family problems, youth unemployment, and social discrimination by reforming laws and designing programs. Mental health policymakers and professionals should create conditions to prevent cigarette smoking initiation.
