Frequency and types of headache and clinical conditions related to cerebral venous hypoplasia in patients referred to Alavi Hospital in 2022: a cross-sectional study

Background: Headache is one of the most common complaints among patients visiting healthcare centers and neurology clinics, significantly affecting quality of life. In the evaluation of headache causes, some patients present with cerebral venous hypoplasia. Although venous hypoplasia is often considered a normal anatomical variation, its clinical relevance remains important. This study aimed to determine the prevalence of different types of headache and the clinical manifestations associated with cerebral venous hypoplasia in patients referred to Alavi Hospital, Ardabil, in 2023.
Methods: This descriptive cross-sectional study was conducted on 95 patients hospitalized in the neurology ward of Alavi Hospital during 2023. All patients underwent MRV, which confirmed the presence of venous hypoplasia. Clinical symptoms including headache, blurred vision, tinnitus, and vomiting were evaluated. Data were analyzed using SPSS version 24 with descriptive and analytical statistical methods.
Results: A total of 95 patients (68.4% female) with a mean age of 45.1 years were enrolled. Migraine headache (43.2%), tension-type headache (35.8%), and vomiting (32.6%) were the most common clinical manifestations. Migraine was significantly more prevalent in patients under 40 years of age, while tension-type headache was more common in those over 60 years (p<0.05).
Conclusion: Cerebral venous hypoplasia may be associated with common clinical presentations such as headache, particularly migraine and tension-type headaches. Although this anatomical variation is frequently considered a normal finding, its association with headache patterns should not be overlooked. The findings highlight the value of MRV imaging as a useful adjunctive tool in the differential diagnosis of headache and other common neurological complaints.
