Risk factors, complications and direct costs of falls in northern Iran: A retrospective study

Background: Falls are a major cause of disability and a major public health concern as our society’s age. Epidemiological studies on falls, particularly those that consider direct hospital costs, have been limited. However the aim of the present study was to determine the risk factors, complications and direct costs of falls in northern Iran.
Methods: In a retrospective study over a three-year period, the files of fall victims were included in the study as a whole. The variables under study were extracted using the hospital's HIS. The data were entered and analyzed in SPSS version 21.
Results: A total of 13,126 patients with a mean age of 37.91±0.42 and a mean length of stay of 2.92±0.028 days were included in the study. 0.4% of patients died. The mean cost per patient was 3,053,514.96±102.61097 Rials. Patients who fell at home were significantly older than patients who fell outside the home, and the mean age of patients who had complications from falls was significantly higher (p<0.001). Men were significantly more likely to fall outside the home (p<0.001). The mean age of patients who fell at dusk and night was significantly lower compared to those who fell during the day, and the mean cost of each recovered patient was significantly lower than that of patients who died. A significant association was also observed between the increase in length of hospitalization and the cost of patients who fell (p<0.001).
Conclusion: This study provides a better understanding of the risk factors for falls in the north of the country, their consequences and costs. The knowledge provided in this study is useful for developing prevention and control strategies for these incidents. Given the observed gender difference, fall prevention programs should focus on indoor activities for older women and outdoor activities for younger men.
