fa روندهای زمانی شیوع و بار بیماری سل در ایران، 1371 تا 1401: یک تحلیل ۳۰ ساله Temporal Trends in Tuberculosis Prevalence and Burden in Iran, 1992–2022: A 30-Year Analysis عمومى General پژوهشي Original سابقه و هدف: همچنان یکی از مهم‌ترین چالش‌های سلامت عمومی است و پایش روندهای بلندمدت آن برای ارزیابی اثربخشی برنامه‌های کنترل بیماری ضروری است. این مطالعه با هدف بررسی روند 30 ساله شیوع و بار بیماری سل در ایران طی سال‌های 1371 تا 1401 انجام شد. مواد و روش ها: این مطالعه اکولوژیک-توصیفی با استفاده از داده‌های مطالعه بار جهانی بیماری‌ها انجام شد. شاخص‌های شیوع و سالهای عمر تطبیق شده بر اساس ناتوانی (دالی) ناشی از سل در ایران، منطقه مدیترانه شرقی و جهان به‌صورت نرخ استانداردشده سنی در هر 100 هزار نفر جمعیت استخراج و تحلیل شدند. برای ارزیابی روندهای زمانی از مدل Joinpoint و شاخص متوسط درصد تغییر سالانه استفاده شد و سطح معنی‌داری 0.05 در نظر گرفته شد. یافته ها: یافته های مطالعه نشان داد دالی و شیوع ناشی از سل با نرخ متوسط تغییر سالانه 50/4-  و 0/39- روندی کاهشی داشته اند (P<0.001). در زیرگروه‌ها، روند دالی و شیوع سل حساس به دارو کاهشی بوده، اما سل مقاوم به چند دارو و سل فوق‌مقاوم به دارو روند افزایشی داشته اند (P<0.001). نتیجه گیری: اگرچه بار کلی سل در ایران طی سه دهه گذشته کاهش یافته است، افزایش انواع مقاوم به دارو نشان می‌دهد که تقویت تشخیص زودهنگام، پایش مقاومت دارویی و درمان هدفمند همچنان ضروری است. Abstract Background: Tuberculosis (TB) remains one of the most significant public health challenges, and monitoring its long-term trends is essential for evaluating the effectiveness of disease control programs. This study aimed to assess the 30-year trend of TB prevalence and disease burden in Iran from 1994 to 2023. Methods: This ecological descriptive study was conducted using data from the Global Burden of Disease study. Indicators of TB prevalence and disability-adjusted life years (DALYs) in Iran, the Eastern Mediterranean Region, and globally were extracted and analyzed as age-standardized rates per 100,000 population. Temporal trends were assessed using the Joinpoint regression model and the average annual percent change (AAPC), with a significance level set at 0.05. Results: The findings indicated a decreasing trend in both DALYs and TB prevalence, with AAPCs of −4.50 and −0.39, respectively (P < 0.001). In subgroups, both DALYs and prevalence of drug-susceptible TB showed declining trends, whereas multidrug-resistant TB and extensively drug-resistant TB demonstrated increasing trends (P < 0.001).
