Factors Associated with the Non-receipt of Healthcare Services among Elderly Individuals Covered by Iranshahr University of Medical Sciences

Abstract
Introduction: Given the increasing elderly population and the negative consequences of inadequate healthcare services, this study aimed to identify the factors associated with the lack of healthcare services among the elderly under the coverage of Iranshahr University of Medical Sciences.
Methods: In this cross-sectional study, data from 2168 elderly individuals were collected using random sampling. The data collection tool was a researcher-designed questionnaire, which had been validated for its reliability and validity.
Results: The findings showed that 61.76% of the elderly lived in rural areas, while 39.23% resided in urban areas. Additionally, 54.90% of the elderly were illiterate. Economic dependency on children and relatives was reported in 42.76% of the elderly. Among the factors affecting the lack of healthcare services, "failure to conduct risk assessments" had the highest mean score of 64.51 ± 26.74, while "lack of trust in healthcare personnel" had the lowest mean score of 31.51 ± 14.91. Delgan County faced the most difficulties in conducting screening tests (66.83 ± 33.61), and Sarbaz County had the least trust in healthcare personnel (41.71 ± 28.60). A significant relationship was also observed between the elderly's economic status and their lack of healthcare visits (P-value < 0.05). Rask County reported the least problems across all areas (21.98 ± 1.78), while Mehrestan (49.81 ± 5.91) and Delgan (49.32 ± 15.88) counties reported the most significant issues.
Conclusion: Factors such as lack of access to risk assessments, inadequate physical conditions of healthcare centers, and economic barriers were identified as the primary reasons for the elderly's underuse of healthcare services. Therefore, it is recommended that policymakers develop and implement comprehensive strategies, including enhancing awareness through educational programs, strengthening the physical infrastructure of healthcare centers, establishing economic support systems, and improving trust in healthcare personnel, to improve elderly access to healthcare services.
